What Is Social Security
In 1935, President Franklin Roosevelt signed the Social Security Act. This new federal legislature started the collecting of taxes in 1937. Since then, working citizens across the United States began paying into social security in order to have a form of guaranteed income once they reached retirement age.
The Social Security Act was initially meant to be a form of basic retirement for working individuals. However, in 1939, an amendment was made to the act that allowed spouses and/or children to begin receiving social security survivor benefits if a parent or spouse was deceased and qualified for social security income.
The Documents You May Need To Provide Include:

- A birth certificate
- Proof of U.S. citizenship or lawful immigrant status
- Discharge papers from the United States military, if you served prior to 1968
- W-2 forms or self-employment tax returns for the previous year
- An Adult Disability Report to provide more details about your condition, as well as your work history
- Medical records, such as doctors reports and test results
- Proof of workers compensation benefits such as settlement agreements or pay stubs
The SSA will accept photocopies of tax documents such as W-2 forms and medical documents. You must however, provide the originals of other documents, such as your birth certificate.
You will also need additional information when applying for disability benefits. The SSA will ask you if you or anyone else has ever applied for benefits on your behalf, whether you earned an income every year since 1978, and if youve ever been convicted of a crime.
The SSA will also ask basic information, such as your name and age, if youre married, and if you have children. If you are married or have children, youll also need to provide the SSA with the names and ages of your spouse and children.

Ceo And Cfo Of Kentucky Medical Companies Sentenced To Prison For Defrauding The Us Ordered To Pay More Than $15m In Restitution
LEXINGTON, KY After a joint investigation by the U.S. Department of Labors Employee Benefits Security Administration and the IRS, U.S. District Court Judge Karen K. Caldwell sentenced two Kentucky healthcare companies executives to prison and ordered them to pay $1,595,725 in restitution.
The court sentenced former Chief Executive Officer Ann Sonderman Giles and former Chief Financial Officer Lu Anne Wallace of Kentucky-registered companies including GW Management Inc., Clark Medical Ventures LLC, Madison Primary Care LLC, Paragon Kentucky LLC and Primary Care of the Bluegrass LLC, for defrauding the U.S. Giles received a sentence of 30 months in prison and 36 months of supervised release, and former chief financial officer Lu Anne Wallace to 33 months in prison and 36 months of supervised release. Both pleaded guilty in June 2020 to one count of conspiracy to defraud the U.S.
As part of their plea agreement, the U.S. government dismissed four counts of an indictment from June 6, 2019, in which a grand jury had charged Giles and Wallace with embezzling health care plan funds in violation of 18 U.S.C. § 669.

A federal jury in Lexington, Kentucky, today convicted a clinical psychologist for his role in a Social Security disability fraud scheme that included a former Social Security Administration administrative law judge and that involved the submission of thousands of falsified medical documents to the SSA, obligating the SSA to pay more than $600 million in lifetime benefits to claimants predicated on these fraudulent submissions.
