Social Security Scotland will:

  • use it as an application for Adult Disability Payment

You do not need to complete a separate Adult Disability Payment application.

Social Security Forms For Disability Benefits

Even if you plan to file online, it is advisable to write out a rough draft on paper forms so that you dont forget to include important information when working online or when you are in a Social Security office filing in person. The base application for Social Security Disability is the SSA-16-BK, while the base application form for Supplemental Security Income disability is the SSA-8000-BK. Base application forms for disabled widows and adult children are the SSA-10 and SSA-4-BK, respectively. These last two forms can be found in the SSA Retirement/Dependent/Survivor Forms section of this website.

Whether you are applying for Social Security Disability as a disabled worker, a disabled widow, a disabled adult child or for SSI as an adult, in addition to the SSA-16-BK or SSA-8000-BK, you also have to complete an SSA-3368 to provide medical information, an SSA-3369 to provide your work history, and an SSA-827, which allows the Social Security Administration to collect information for the purpose of determining your eligibility. You may also want to use some of the other forms available in this grouping of forms to submit additional information about your limitations or special accommodations.

Please click on the link below to open your desired form in a separate browser window. You can then download the form from DisabilityResources.org directly to your computer.

If You Get Pip Or Dla For Adults And You Apply For Adult Disability Payment

You do not need to make a new application for Adult Disability Payment if you already get Personal Independence Payment or Disability Living Allowance for adults from the Department for Work and Pensions .

Social Security Scotland will move your benefit to Adult Disability Payment without you having to do anything.

You cannot get PIP or DLA for adults and Adult Disability Payment at the same time.

If you do make a new application for Adult Disability Payment and Social Security Scotland have started to move your benefit then theyll:

  • accept Part 1 of the new application as a valid application for Adult Disability Payment
  • make a determination using Part 1 of the new application only

A determination is the legal term for the decision Social Security Scotland makes about your Adult Disability Payment.

Once they have made the determination on your new application, Social Security Scotland will then continue with the process of moving your benefit to Adult Disability Payment.

Until the move happens, youll continue to get PIP or DLA for adults from the DWP.

Visiting Or Calling Your Local Social Security Office To Apply

Social Security Disability Application Form Ct

While applying online is convenient, another method to complete the entire application process is by visiting your local social security office.

The closest social security office can be contacted by phone in order for an initial interview to be scheduled.

In-person and phone interviews are available at many offices.

You can find out your local SSA office and its contact information by using the SSAs Find An Office Tool.

In-person interviews allow SSA employees to ask questions that help them determine whether an applicant is eligible for benefits.

While an employee can identify basic qualification criteria , the ultimate is based upon a review of all evidence of your disability.

You should note, however, that the employee taking your application will also make observations about your appearance, as well as your ability to answer questions and complete the interview. These observations will be made a part of the claim file.

Phone interviews also give SSA employees a chance to determine which forms an applicant will need to fill out in order for their application to be considered complete.

These forms are then mailed to the applicant for completion.

While this makes it easy for applicants to understand what forms are necessary, the forms themselves often seek important information in less than clear terms.

Question 2: Intent To File For Other Benefits

SSA needs to know if the applicant intends to file for any other benefits. For SOAR, representatives assist with both SSI and SSDI applications, so the box for Supplemental Security Income should be checked. If the applicant served in the military, check the box for Veterans Administration Benefits and ensure that the applicant is connected with assistance for VA disability benefits. If the applicant is eligible for welfare in the state he or she resides, check that box as appropriate.

The Basics About Disability Benefits

The SSDI program pays benefits to you and certain if you are insured. This means that you worked long enough and recently enough – and paid Social Security taxes on your earnings. The SSI program pays benefits to adults and children who meet our requirements for a qualifying disability and have limited income and resources.

While these two programs are different, the medical requirements are the same. If you meet the nonmedical requirements, monthly benefits are paid if you have a medical condition expected to last at least one year or result in death.

What You Need To Know When Applying For Disability Online

If you prefer to apply for Social Security Disability benefits from the comfort of your home, there are a few things you need to know. First and foremost, before you sit down to fill out the Social Security Disability application, make sure you have access to all of the information you will need in order to apply for disability.

This information includes details about your identity as well as information about the medical basis of your Social Security Disability claim. You will also need information about your earnings for the previous year and the current year as well as your Social Security Earnings Statement.

Finally, you may want to have your bank account information on hand if you want your Social Security Disability benefits deposited electronically to your bank account.

Once your online application for SSDI has been completed, you will need to print out some forms and send them to the Social Security office. These forms include the Adult Disability Report and the necessary medial release forms.

These forms cannot be filled out online and must be sent into the office. If you do not mail these forms to the SSA, the Social Security office will not be able to process your application for Social Security Disability benefits and your claim will be denied.

The Online Application Process

A convenient option if you have access to the Internet is to begin the application process online.

The SSA website allows you to start the disability application process by providing basic personal information.

However, it is important to remember that medical records and other proof of eligibility status will need to be provided to and/or obtained by the SSA before an application is considered complete.

Applying online is simply a convenient starting point for the entire process. The only place to file your application online is www.SSA.gov.

Let me offer a warning there are many sites that appear official but are not associated with the Social Security Administration.

These sites will have you believe you are submitting an application for disability benefits to SSA, but you are really giving your personal information to a third party who will then attempt to have you sign a contract with them to represent you.

Please be sure that you know the difference and are making an informed choice. The only sites directly administered by the Social Security Administration are www.ssa.gov and www.socialsecurity.gov.

Please note that SSI or Title XVI applications are not accepted over the internet at this time, and you must make this application directly with your local office.
