The Department for Work and Pensions will send your Personal Independence Payment application to Social Security Scotland.

Social Security Scotland will:

  • use it as an application for Adult Disability Payment
  • make a decision about your application

You’ll get your payments from the date you applied for Personal Independence Payment , if your application is successful.

You do not need to complete a separate Adult Disability Payment application.

Social Security Forms For Disability Benefits

Even if you plan to file online, it is advisable to write out a rough draft on paper forms so that you dont forget to include important information when working online or when you are in a Social Security office filing in person. The base application for Social Security Disability is the SSA-16-BK, while the base application form for Supplemental Security Income disability is the SSA-8000-BK. Base application forms for disabled widows and adult children are the SSA-10 and SSA-4-BK, respectively. These last two forms can be found in the SSA Retirement/Dependent/Survivor Forms section of this website.

Whether you are applying for Social Security Disability as a disabled worker, a disabled widow, a disabled adult child or for SSI as an adult, in addition to the SSA-16-BK or SSA-8000-BK, you also have to complete an SSA-3368 to provide medical information, an SSA-3369 to provide your work history, and an SSA-827, which allows the Social Security Administration to collect information for the purpose of determining your eligibility. You may also want to use some of the other forms available in this grouping of forms to submit additional information about your limitations or special accommodations.

Forms Related To Social Security Disability

Here are some specific forms that must be filled out by every disability benefit applicant. These forms are used to evaluate the eligibility of each individual applicant.

Form SSA-3368-BK: Adult Disability Report you provide all of the relevant information concerning your medical conditions, medical doctors and treatment, medications, how your daily activities are affected by your disability, your educational background, and other information.

Form SSA-3369-BK: Work History Report you provide specific information on your jobs and what type of function was required of you in each job. This is used in determining what your past relevant work experience is, whether you can perform your past relevant work with your disability, and to determine what transferable skills, if any, you may have.

Form SSA-3373-BK: Adult Function Report you provide specific information about how your medical condition affects your ability to perform tasks and function.

Form SSA-827-BK: Authorization to Disclose Information to the Social Security Administration you must sign and return this form so that SSA can obtain your medical records which are used to determine the extent of your disability.

If You Get Pip Or Dla For Adults And You Apply For Adult Disability Payment

You do not need to make a new application for Adult Disability Payment if you already get Personal Independence Payment or Disability Living Allowance for adults from the Department for Work and Pensions .

Social Security Scotland will move your benefit to Adult Disability Payment without you having to do anything.

You cannot get PIP or DLA for adults and Adult Disability Payment at the same time.

If you do make a new application for Adult Disability Payment and Social Security Scotland have started to move your benefit then theyll:

  • accept Part 1 of the new application as a valid application for Adult Disability Payment
  • make a determination using Part 1 of the new application only

A determination is the legal term for the decision Social Security Scotland makes about your Adult Disability Payment.

Once they have made the determination on your new application, Social Security Scotland will then continue with the process of moving your benefit to Adult Disability Payment.

Until the move happens, youll continue to get PIP or DLA for adults from the DWP.
